Form No. 1 PETITION FOR DEGREES Used only for a new petitioner or one having been previously rejected. All information requested should be answered, otherwise it is not complete and ready to be acted upon. All information on this form should be made available to the investigation committee when requested. Names of all applicants using Form No. 1 are to be reported to the Grand Secretary on Form No. 108. (Sec. 229 and Sec. 111-2) Form No. 2 PETITION FOR AFFILIATION Petitions for affiliation may be received according to the requirements of the Code. It is extremely important that this Form be completely filled out by the applicant, and that the information received in this manner be transmitted to the Grand Secretary on the Monthly Report. This form must always be accompanied by a Certificate or Letter of Good Standing issued by the applicants' Lodge. A Dues Card is not sufficient. (Sec. 234) Form No. 3 PETITION FOR REINSTATEMENT Should be used by those who, having been members in good standing, have been dropped from the rolls in the manner prescribed in the Code. The same importance attaches to this Form as in all other petitions that all questions be answered. (Sec. 255-4) Form No. 3-A PETITION FOR RESTORATION Used for petitioning a Lodge by a person who has been expelled and wishes to be restored to membership. (Sec. 319-1, 354 and 386) Form No. 4 PETITION FOR ADVANCEMENT Used by the individual whose petition for the degrees has been approved by the Lodge and who may have been dropped for failure to present himself within the prescribed time either before or after receiving the E.A. or F.C. Degree. (Sec. 235 and 236) Form No. 5 PETITION FOR DUAL OR PLURAL MEMBERSHIP FROM OUTSIDE STATE Used when the petitioner is a member, in good standing, in a Jurisdiction permitting dual membership and is recognized by the Grand Lodge of Oregon. This petition must be sent to the Grand Secretary for his certification before it may be legally proceeded with by the Lodge. If the petitioner has never been a member of an Oregon Lodge, he must pay the affiliation fee fixed by the Code. A current Certificate or Letter of Good Standing must accompany this petition. (Sec. 226 and Sec. 157-8) Form No. 5 PETITION FOR DUAL OR PLURAL MEMBERSHIP FROM INSIDE STATE Used where the petitioner is a member in good standing in a constituent Lodge of Oregon and desires dual or plural membership with another Oregon Lodge. He does not pay the affiliation fee. (Sec. 226 and Sec. 157-8) Form No. 6 COMMITTEE REPORT - DEGREES It is important that this Form, when given to the Committee, have the information clearly stated as evidenced by the petition. It is also extremely important that the questions in the report be fully answered before being received by the Lodge. (Sec. 237) Form No. 7 REPORT ON AFFILIATION Contains information that can be had usually only at the time that the applicant applies for membership. For this reason all questions should be fully answered. (Sec. 237) Form No. 8 REPORT ON REINSTATEMENT Used by the Investigating Committee on the petition of a Brother for reinstatement. (Sec. 237) Form No. 9 REPORT ON ADVANCEMENT Used by the Investigating Committee on the petition of a candidate for advancement. (Sec. 237) Form No. 10 NOTICE TO CANDIDATE OF ELECTION TO RECEIVE DEGREES Used by the Secretary to notify the candidate of his election to receive the Degree in Masonry, and to indicate to him the time and place for his first Degree. Form No. 11 NOTICE TO APPEAR FOR DEGREES Used by the Secretary as a notice to the candidate to appear, giving time and place, for the purpose of having a degree conferred upon him. Form No. 12 NOTICE OF ELECTION TO MEMBERSHIP BY AFFILIATION Used by the Secretary to notify the applicant that he has been elected to membership in the Lodge by affiliation and that he should appear to sign the By-Laws. (Sec. 213-19) Form No. 13 NOTICE OF ELECTION TO MEMBERSHIP BY REINSTATEMENT Used by the Secretary to notify the petitioner that he has been reinstated to membership in the Lodge. Form No. 14 CERTIFICATE OF PROFICIENCY Used to notify the candidate that he has passed a satisfactory examination on the Candidate's Lecture of the Master Mason Degree. It is to be signed by the Worshipful Master, attested by the Secretary under the seal of the Lodge. Form No. 15 CERTIFICATE OF MEMBERSHIP Issued to the Candidate after he has received his Master Mason Degree. It gives the dates of each degree and is signed by the Master, attested by the Secretary under the seal of the Lodge. The member affixes his signature to this document. Required by the Code Sec. 213-20. Form No. 16 NOTICE OF REJECTION Used by the Secretary of the Lodge to notify the Petitioner that his Petition for the degrees of Masonry was rejected. (Sec. 239) Form No. 17T TRANSFER CERTIFICATE (In State Only) Used in all instances where a member is transferring his membership from one Oregon Lodge to another. He does not want to continue membership in the Lodge he is coming from. This Form is in three parts, and gives notice to the Grand Secretary of the Lodge to which the Brother signifies his interest to transfer, and enables the Grand Secretary's office to notify that Lodge of such proposed transfer. Do Not Drop a member from your Lodge rolls until notice is received, from the Grand Secretary's office, of his election to membership in another Lodge. (Sec. 224-1 (c), Sec. 247 and Sec. 256-8) Form No. 17D DUAL MEMBERSHIP (In State Only) Used in all instances where a member is affiliating from one Oregon Lodge to another Oregon Lodge. He desires to remain a member in both Lodges. Form No. 18 CERTIFICATE OF TRANSFER OUT OF STATE Issued by the Secretary to the member of his Lodge who wishes to transfer his membership to a Lodge outside the Jurisdiction of Oregon. The Request for Demit is a part of this form and should remain attached when issued. Form No. 19 DEMIT Issued by the Secretary to a member of his Lodge whose residence is outside the Jurisdiction of Oregon, and who applies for the demit by letter and must be recorded in the Minutes. If the member is a resident of Oregon, issue Form No. 20. (Sec. 248, Sec. 249 and Sec. 256-9) Form No. 20 CERTIFICATE OF WITHDRAWAL This instrument is issued to a member who has made written request for it, and such action by the Secretary does not require the approval of the Lodge, but must be reported at the next Stated Communication and be included in the Monthly Report to the Grand Secretary. Dues must be paid up to date, before certificate is issued. (Sec. 250 and Sec. 256-9) Form No. 20A CERTIFICATE OF SEVERANCE OF PLURAL MEMBERSHIP Issued to a Dual or Plural member who has made written request to withdraw or sever membership. Handled same as Form No. 20. Form No. 21 LODGE CERTIFICATE Issued by the Secretary to a member who needs evidence of Master Mason standing, in order that he may obtain dual or plural membership in a Jurisdiction other than Oregon, which permits such membership, or as evidence of Masonic standing on a particular date. Form No. 22 FAMILY CERTIFICATE - DEATH CERTIFICATE Issued to the family of a deceased Brother as evidence of his Masonic standing in the Lodge of which he was a member at the time of his death. The issuance of this Certificate is required by Code Sec. 213-30. Form No. 23 NOTICE OF ANNUAL ELECTION AND DUES Automatically sent to the Lodge Secretary after processing by computer to notify the members of his Lodge of the time of the Annual Election of Officers and the amount of current and delinquent dues owing. Form No. 23 DUES NOTICE Used by the Secretary to notify his members, as required by the Code, of dues that are delinquent and unpaid. (Sec. 255-1 (b) Form No. 24 NOTICE TO DELINQUENTS Used by the Secretary to notify those members of his Lodge who are delinquent and subject to being dropped by the Lodge, as provided by the Code. (Sec. 255) Form No. 25 NOTICE TO DROPPED FROM ROLL Used by the Secretary to notify those members of his Lodge who have been dropped for non-payment of dues. (Sec. 255) Form No. 26 MASTER MASON LECTURE NOTICE Used to give notice to the candidate who has received the Master Mason Degree, that he will be denied all the rights and privileges of Masonry, if the examination on the Candidate's Lecture of the Master Mason Degree is not passed within the prescribed time. (Sec. 244-1(e) Form No. 27 REQUEST FOR WAIVER OF JURISDICTION Used by a Lodge other than the one in which he petitioned or received the degree or degrees, requesting Waiver of Jurisdiction of candidate who has petitioned the Lodge for the degrees or for advancement. See Code for law on this matter. (Sec. 230) Form No. 27 WAIVER OF JURISDICTION Used by a Lodge granting a Waiver of Jurisdiction on a candidate who has petitioned another Lodge for the degrees or for advancement. See Code for law on this matter. (Sec. 230) Form No. 28 REQUEST FOR COURTESY WORK Must be used by the Lodge requesting another Lodge in a Jurisdiction, other than Oregon, to confer degrees by courtesy. Identification for the candidate is removed, sent to him, and the Form mailed to the Grand Secretary of Oregon for processing. Further correspondence on the matter will be carried on between the two Lodges concerned. (Sec. 245-2) Form No. 29 APPLICATION FOR LIFE MEMBERSHIP Must be used in all cases by the Secretary where a Brother has paid the fee prescribed in the By-Laws of the Lodge for the purchase of a life membership. When received in the Grand Secretary's office, the pocket card and certificate will be prepared. The cost of such preparation is charged to the Lodge. The Secretary will receive a receipt indicating the number of the Life Membership. A certificate and pocket card will be mailed to the address indicated on the application. This Form is also used for the Memorial Life Membership. The only additional information required in this office is the date of the death of the Brother and the name of the Donor. Form No. 30 APPLICATION FOR EDUCATIONAL ASSISTANCE Used by the Lodge applying for educational assistance for children or grandchildren of deceased or disabled Master Masons who are attending public schools. Grades K through 12, or post high school through age 22. Form No. 30A REQUEST FOR APPLICATION FOR EDUCATIONAL ASSISTANCE Used by Lodge to obtain an application for a specific child or grandchild. Form No. 31 25 YEAR MEMBERSHIP CERTIFICATE Used when a Lodge wishes to recognize a member attaining 25 years of good standing. Form No. 32 APPLICATION FOR 50 YEAR EMBLEM Used for the purpose of applying for a 50 year emblem to be presented to a Brother who has been a Master Mason in good standing for a period of 50 years or more. The Code does not permit presentation prior to the completion of fifty years membership in good standing. Form No. 33 REPORT OF PRESENTATION OF 50 YEAR EMBLEM Executed following the presentation of the 50 year emblem and immediately returned to the Grand Secretary's office, as this item is included in the proceedings of the Grand Lodge. A copy of this form will accompany each 50 year emblem issued. Form No. 34 LIFE MEMBERSHIP CERTIFICATE Issued following receipt of the application from the Lodge and the fee as prescribed in the Lodge By-Laws. It is mailed to the Lodge Secretary or the individual as designated on the application. The charge for its preparation is billed to the Lodge. (No. 212 - Frames, as listed on the price list of supplies, are available.) Form No. 34A MEMORIAL LIFE MEMBERSHIP CERTIFICATE Same as above except for deceased members. Form No. 35 PAST MASTER CERTIFICATE Issued upon the request of the Lodge for the Past Master designated. The cost of the preparation is charged to the Lodge. Gives the Lodge name and number, the year served and is signed by the Grand Master and attested by the Grand Secretary. Will fit in No. 212 Frame. Form No. 35A PAST MASTER - POCKET CARD Form No. 35B HONORARY PAST MASTER CERTIFICATE Form No. 36 FUNERAL ODE CARDS This is a heavy pasteboard card on which is printed the funeral ode. Form No. 38 TRIAL SUMMONS Used by the Secretary to summon a Brother to a Masonic trial. Form No. 39 LODGE SUMMONS Used by the Secretary by order of the Worshipful Master to summon members to attend a communication of the Lodge. Form No. 42 APPLICATION FOR EXAMINATION FOR PROFICIENCY ON SECRET WORK Supplied by the Chairman of the Grand Lodge Committee on Masonic Instruction, the Grand Lecturer, to the member of the Lodge who wishes to take the proficiency examination on the ritual in use by this Grand Lodge. Form No. 48 FORM OF MINUTES A guide to assist the Secretary in writing the minutes. Form No. 50 UNRULED MINUTE SHEETS WITH HEADING The Form adopted by the Grand Lodge for the use of the Secretary in recording the minutes of the Lodge. This Form has the printed heading on the sheet. Form No. 51 MINUTE SHEETS - BLANK A blank sheet to be used for recording minutes, and is unruled. This is also used for a continuation sheet when more than one page is needed. Form No. 52 RULED MINUTE SHEETS WITH HEADING This Form has the heading for the recording of minutes of the Lodge, and is ruled for longhand writing. Same as No. 50 except it is ruled. May also be used by the Secretary as a work sheet for recording the minutes of the Lodge during the time the Lodge is in session. Form No. 55 LEDGER SHEETS Suggested as a proper ledger sheet upon which to record the financial transactions relative to membership of each member. These fit No. 73, Ledger Binder. (Sec. 221-6 (a&b) Form No. 55A LEDGER CARDS A 5x8 inch card with same information as Form No. 55 Form No. 57 TEMPORARY DUES CARD This Dues Card, in sheets of 6, is secured from the Grand Secretary's office and covers a period of time provided in the Code, in which the Master Mason must prove his proficiency in the Candidate's Lecture of the Master Mason Degree. Often referred to as the six months dues card and should be the only dues card issued until member passes Candidate's Lecture on M.M. Degree. (Sec. 224-1(d) Form No. 57A YEARLY DUES CARD This card is the Dues Card authorized by the Grand Lodge and supplied by the Grand Secretary based upon the membership of the Lodge and is printed by the computer. Extra blanks will be furnished upon request. (Sec. 213-21) Form No. 58 CARD MEETING NOTICE May be ordered from the Grand Secretary's office and used as a notice to the members of the Lodge meetings. It is a postal card on the back of which various dates are indicated. Form No. 59A NOTICE TO GRAND SECRETARY This is a Form to be used to notify the Grand Secretary of another Grand Jurisdiction that a member of your Lodge has moved to that Jurisdiction. (Return to the Grand Secretary's office.) Form No. 60 SECRETARY'S RECEIPT BOOK A receipt book for the Secretary to use in acknowledging payments to him, for the Lodge, of money other than for dues. (Sec. 221-4 (a) Form No. 61 TREASURER'S RECEIPT TO SECRETARY - BOOK Used by the treasurer to receipt to the Secretary for moneys received from him. (Sec. 220-2) Form No. 62 WARRANTS ON TREASURER - BOOK An order signed by the Master and Secretary authorizing the Treasurer to issue a check for a specified item. Form No. 63 SECRETARY'S CASH BOOK A loose leaf ledger consisting of 25 receipt pages and 25 disbursement pages having a two post canvas cover with post extensions included. Extra pages may be ordered as needed (No. 63A and 63B) a dozen or more at a time. Form No. 65 TYLER'S REGISTER - BOUND Bound books of members and visitors registration sheets. Form 65 contains 200 sheets. Form No. 67 TYLER'S REGISTER - LOOSE LEAF PAGES Pages for the Tyler's Register Loose Leaf Binder may be purchased by the dozen. Form No. 68 WORK SHEET To be used in recording all transactions effecting the membership of the Lodge. If kept in the front of the minute book, it is always available to make proper entries at the time a change is effective. The Monthly Report is made up by sing the information recorded on the Work Sheet for the current month. The Work Sheet is also used to verify the Annual Report (70A) when received. The Work Sheet is part of the pad of report forms that the Secretary will receive shortly after January 1st each year and need not be ordered and is to be retained in the Lodge files. Form No. 69 MONTHLY REPORT DUE IN GRAND SECRETARY'S OFFICE BY THE 10TH OF THE FOLLOWING MONTH Monthly Reports are supplied by the Grand Secretary's office in the pad of required report forms mailed, without ordering, about January 1st each year. These forms are supplied in duplicate, one to be sent to the Grand Secretary and one to be retained as Lodge records. You will not find report forms for the months of July and August in this pad. Any activity for these two months (deaths, withdrawals, etc.) should be reported on the September report. This report is one of the most important used in the work of the Secretary and Grand Lodge record keeping. The information recorded on this report becomes the permanent record of the individual member on all Grand Lodge records. The first information concerning a candidate receiving the degrees reported to the Grand Secretary's office on the Monthly Report should be the date the E.A. Degree is conferred and at that time fill in all information concerning name, birth date and place. Transfers from one Oregon Lodge to another should not be recorded until the notice has been received from the Grand Secretary's office giving the date the Brother was elected to membership in the new Lodge. Always be sure to note, if the membership is Dual or an outright transfer from one Lodge to another. When reporting a candidate receiving the M.M. Degree, the current address of the member should be given in order to get this information into the computer program. DO NOT USE THIS FORM TO REPORT CHANGES OF ADDRESS. THESE SHOULD BE ON THE CHANGE OF ADDRESS FORM SUPPLIED BY THE GRAND LODGE. (Sec. 213-13, 217-12 and 221-11) Form No. 70A ANNUAL RETURNS Information reported on the ten Monthly Reports by the Secretary is accumulated on this form and after completion following the receipt of the December Monthly Report, is the basis for computing the billing of per capita dues for each Lodge. Two copies of the Annual Returns are sent to the Secretary prior to March 31st each year for verification as to information recorded thereon. One copy is signed by the Secretary and Master before returning it to the Grand Secretary. The other signed copy is for Lodge files. If the information is not correct, report the differences to the Grand Secretary, using a separate sheet of paper, do not alter the form, for correction or clarification. Information not reported on Monthly Reports should be entered on the next Monthly Report and adjustments in billing will be made the following year. If information reported is not included, send the Annual Return forms back to the Grand Secretary for correction. In other words, if it is your error, report it on the next Monthly Report and, if it is the Grand Secretary's error, it will be corrected for the year being considered. The Grand Lodge copy of this report is DUE BY APRIL 10TH. If not received, a penalty of $1.00 per day is assessed. (Sec. 213-8-9-10, Sec. 217-12 and Sec. 221-11) Form No. 70B MEMBER RELIEVED FROM DUES A part of the Monthly Report pad and found between the reports for November and December. Only a member, "unable to pay his dues", is eligible as provided in the code. To remit dues for the reason a member is a 50 year member, Secretary, or custodian is not sufficient reason to remit dues in respect to this form. He must be unable to pay. This report is due, as noted at the bottom of the form, with the December Monthly Report by January 10th. If not received by this date, it may be too late to receive credit. The Lodge may vote at any Stated Communication to remit dues of a member for the current year, and when reporting at the end of the calendar year, only those remitted for that year should be reported. Do not report dues being remitted for the next year as this will not be allowed until figuring Grand Lodge pr capita billing at the end of that year. (Sec. 254-3) Form No. 70½ OFFICER'S LIST Also included in the Monthly Report pad and found between the reports for November and December. As noted on the form, this should be sent immediately following the election of Officers. This form is needed as soon as possible in order that the Directory of Lodges may be printed and distributed. On the prompt submission of this report depends how soon the new list of Lodge Officers will be printed and mailed. DO NOT WAIT until after installation of Officers to mail this report. (Sec. 213-16) The report of installation may be sent later. Form No. 71 REPORT OF AUDITING COMMITTEE Used by the Committee appointed by the Master to audit the records of the Lodge. This form is also included in the pad of Monthly Reports. One copy is to be sent to the Grand Secretary and one retained for the Lodge records. (Sec. 217-19) Form No. 72 FINANCIAL REPORT Contained in the pad of Monthly Reports and, when completed, is a detailed statement of the receipts and disbursements of the Lodge. It also contains information regarding the amount of unpaid dues, as well as the investments held by the Lodge, and includes the debts and other pertinent information. Form 72 should be accompanied by the Report of Auditing Committee (Form No. 71) when filed with the Grand Secretary. (Sec. 217-19 (c) Form No. 73 LEDGER OR MINUTE SHEET BINDER Used for maintaining a membership ledger and is also used to bind or contain the minute sheets of the Lodge. These are the canvas covered (key lock or slide lock) type binder that Form 55 and minute sheets fit. Form No. 100 PETITION FOR DISPENSATION TO FORM A NEW LODGE Adopted by the Grand Lodge to be used when a group of Masons wish to petition the Grand Master for a dispensation to form a new Lodge. (Sec. 258) Form No. 104 DISPENSATION FOR A NEW LODGE Issued by the Grand Master to a group of Freemasons sponsored by a regular Lodge in this Grand Jurisdiction for the purpose of functioning as a Lodge until such time as the Grand Lodge may issue them a charter, or their Dispensation be discontinued. (Sec. 258) Form No. 105 PETITION FOR A CHARTER Used by a Lodge under Dispensation to petition the Grand Lodge for a Charter. (Sec. 260) Form No. 106 CHARTER OF A LODGE Granted by vote of Grand Lodge in Annual Communication enabling a Lodge to function as such in this Grand Jurisdiction. Form No. 107 BY-LAWS OF A LODGE The By-Laws of the Lodge as prescribed in the Code, Sec. 272, and Form 107, is a copy of the Code in that respect. Forward original and 2 copies to Grand Secretary for the approval of the Grand Master. One copy to be retained in the Grand Secretary's office, one copy will be returned to the Lodge, and one copy for use by the Committee on Jurisprudence. This is the proper form to be used when a Lodge desires to adopt a new set of By-Laws rather than make several amendments to their old set. (Sec. 272) Form No. 108 REPORT OF PETITIONS RECEIVED This report is due by the 15th of each month and is the source of information used in the "List of Petitions" sent to all Lodges each month. ONLY PETITIONS FOR DEGREES should be reported on this form. In listing occupations you should be as specific as possible and if "Retired" give "what" the petitioner is retired from. (Sec. 111-2 and 221-3) Form No. 110 MEMBERSHIP RECORD CARD May be used in setting up a card file of the membership of the Lodge. This 4x6 inch card, when completed, contains name, birth date and place, degree dates and other information comprising the membership record of the member. Form No. 111 ORDER CARDS FOR SUPPLIES May be used as a post card for ordering supplies from the Grand Secretary. Usually you will receive one or more of these with each order of supplies, therefore, you need not order them. Form No. 113 CHANGE OF ADDRESS FORMS Form No. 200 RITUAL (SECRET WORK) Contains the key to the ritualistic work of Oregon. This is not the Code. The Grand Secretary's office has the complete Ritualistic work written out and it is available for study by Master Masons. This book is a must for anyone desiring to learn the Ritual. Form No. 201 OFFICERS HANDBOOK A necessary tool for all officers. Contains information that will lead to good Lodge management, if followed. Form No. 202 COACHING LECTURES A set of three books containing the "key" to the candidates lectures of the three degrees. These are available in sets or for each degree. Form No. 203 STANDARD MANUAL This book is to be presented to each Master Mason at the time of Raising. It contains various ceremonies and non-secret work of the three degrees. Prayers, charges, installation of Officers and other instructions relative to conducting the affairs of the Lodge are to be found in this Manual. (Sec. 213-20) Form No. 204 MASONIC CODE OF OREGON The Constitution and By-Laws of the Grand Lodge and also contains the Penal Code. Each Lodge is required to furnish the Worshipful Master, Senior Warden, Junior Warden, Senior Deacon and Secretary with a copy of the Code. (Sec. 213-33) Form No. 204B BINDER Black or Blue (3-ring, 9"x7½") Form No. 205 BIBLE PRESENTATION Form 205A BIBLE HEIRLOOM Form No. 206 EDUCATIONAL LECTURES Four booklets which are a part of the Mentor Kit. These may be purchased in sets or individually. Form No. 216 INVESTIGATING COMMITTEE INSTRUCTIONS A small booklet to be sent with the Form No. 6 "Committee Report - Degrees". This booklet assists the member in making the investigation of a candidate by outlining some methods which may be used in this important duty. Form No. 218 COACHES HANDBOOK An aid and outline to assist those members coaching candidates in the various lectures. The material contained in this will help the coach answer many questions asked by the candidate. Form No. 219 OREGON MENTOR MANUAL The backbone of the Mentor Kit. A great deal of educational material contained in this book will explain much about Masonry and its teachings. Form No. 220 ONE HUNDRED ONE QUESTIONS ABOUT FREEMASONRY Will answer many questions regarding Masonic History, Grand Lodge, Lodge Degrees, Public Contact and General Questions. Form No. 221 POCKET MASONIC DICTIONARY Gives the definitions of many Masonic terms, names and abbreviations as well as some proper pronunciations. This booklet is a part of the Mentor Kit. Form No. 222 POCKET ENCYCLOPEDIA OF MASONIC SYMBOLS Contains explanations of many of our Masonic symbols which are indexed. This booklet is also a part of the Mentor Kit. Form No. 224 MENTOR KIT Contains the four Educational Lectures (No. 206), a Mentor Manual (No. 219), a Pocket Masonic Dictionary (No. 221) and a Pocket Encyclopedia of Masonic Symbols (No. 222).
